In case you filled out the form and want to change it, but haven't been back to the main Yuletide page to see this note:

If you change your mind before November 9, you're welcome to return and fill out this form again. However, please use the same e-mail address and name, as otherwise we can't easily tell that it's a duplicate. We will use the most recent submission when making the assignments.
